All clients must fill out the Hands On Wellness Health History Form, Appointment Policy Form and Day of Appointment Wellness Form.

The Health History form provides info on what you've experienced throughout life changes. Goes in-depth on your health to make sure you can safely, effectively and medically get massaged. This form can be emailed to you and filled out before your appointment.

The Appointment Policy form keeps you informed on what's expected for basic scheduling and visits. This for can be emailed before your appointment as well.

The Day of Appointment form is to indicate how you're feeling that day and any concerns that brought you in for a massage. This form can only be filled out that day,
